K21 Foundation is interested in finding out what the community knows about its organization and its place in the community. The foundation is asking Kosciusko County citizens to participate in a short survey to let them know how familiar they are with K21 and the many projects and programs the foundation has supported in the past nine years.
People who are interested in participating in the survey should visit www.k21foundation.org and take an online survey, which will take about five minutes to complete.
K21 Foundation exists for the benefit of Kosciusko County citizens to ensure health care services are provided and advance prevention and healthy lifestyles.
