A local nurse has been arrested after detectives say she used a stolen prescription pad to write fake pill orders for a South Bend man.
Jakayla Lynn Snodgrass, 23, of 386 EMS T26 Lane, Leesburg, a registered nurse at Provident Family Healthcare in Warsaw, faces charges of forgery and obtaining a controlled substance – the fallout of a prescription drug fraud scheme that several police agencies caught wind of in late June.
It was then, June 28, that a South Bend man - 28-year-old Thomas Calhoun - walked into a CVS pharmacy in the town and presented a written prescription for Roxicodone, a controlled substance painkiller and a derivative of the popular Oxycodone. The prescription was reportedly written by Snodgrass and without authorization from the office of Dr. David Darr.
Calhoun then fled the store after pharmacists looked over the fake order, police said.
The incident launched a multi-county investigation into the scheme by the Warsaw Police Department and South Bend and St. Joseph County police.
Calhoun was arrested July 22 in South Bend, booked into the St. Joseph County Jail on forgery and attempted prescription fraud charges before being transferred to the Kosciusko County Jail to faces those charges. He posted a $25,000 bond Tuesday.
Snodgrass turned herself in at the Kosciusko County Jail midday Wednesday, and promptly posted her own $25,000 bond.
Detectives continuing to research the scheme have learned that “numerous” similar pill orders have been passed at South Bend pharmacies in the past nine months, carrying both Calhoun’s and Snodgrass’ name. Calhoun was arrested July 12, as well, in South Bend after trying to fill a fraudulent order from Provident Family Healthcare, police said.
Detectives said additional charges are likely to come down on both Calhoun and Snodgrass in relation to other incidents in both counties.
