Dexter Wuthrich, 11, of North CR 200W, suffered head injuries, a broken left arm and a shoulder injury, according to a Department of Natural Resources report.
Wuthrich was riding the four-wheeler in a cornfield, where he was reportedly found unconscious.
He is a patient at Parkview Hospital, Fort Wayne.
The accident occurred at 6 p.m. Saturday.
