A woman accused of forgery and attempting to acquire a controlled substance stemming from a July 2011 incident has pleaded guilty in Kosciusko Superior Court I.

Jakayla Snodgrass, 24, of 386 EMS T 26 Lane, Leesburg, a registered nurse at Provident Family Healthcare in Warsaw, pleaded guilty on Aug. 22 to attempting to acquire a controlled substance. Snodgrass was charged with forgery, a Class C felony, and attempting to acquire a controlled substance by fraud, a Class D  felony.

As per her plea deal, if Judge  Duane Huffer accepts the deal, there will be no limitation as to the court’s authority to impose a sentence in accordance with a Class D felony. The state also agreed to drop the forgery charge as a part of the plea.

In addition, if Snodgrass successfully completes probation, the state agreed to modify the sentence to a Class A misdemeanor. The sentence for a Class D felony can range from six months to three years; the advisory sentence is 1-1/2 years.

On June 28, Thomas Calhoun, 28, South Bend, walked into the Warsaw CVS pharmacy and gave the clerk a prescription for Roxicodone, a controlled substance painkiller. The prescription was written by Snodgrass and without the permission of Dr. David Darr.

The incident sparked a multi-county investigation involving Kosciusko and St. Joseph counties.

Detectives found several other similar instances at South Bend pharmacies during the nine months prior to the Warsaw CVS incident.

Snodgrass turned herself into police July 27, 2011, and posted $25,000 bond.

Snodgrass is next due in court Sept. 20 for plea deal acceptance or denial.
