Drug Store Employee Arrested In Drug Case

Posted

A drug store employee was arrested Thursday after allegedly fraudulently filling a prescription.

Jeffery Steven Van Baalen, 43, of Peru, was taken into custody at the CVS store in Warsaw. He allegedly had a prescription for Hydrocodone filled Wednesday after it was called in for 40 tablets with four refills.

When the pharmacist took the information via telephone, he apparently failed to write the Drug Enforcement Administration number on the prescription. When he later contacted the doctor listed on the prescription, the CVS pharmacist learned that the doctor did not authorize the controlled substance for Van Baalen.

According to a Warsaw Police Department report, Van Baalen was taken into the office of CVS, where management told him he was terminated, then detained him for police.

Indiana State Police are investigating a similar incident in Peru, police said.

Van Baalen was booked into the Kosciusko County Jail on a charge of possession of a controlled substance. He was released after posting a $3,000 bond.
