William D. Boaz, M.D.

SYRACUSE - William D. Boaz, M.D., 78, of Leesburg, died at 4:54 a.m. Jan. 5, 2010, in Kosciusko Community Hospital, Warsaw.

He was born July 27, 1931, in New Albany, Ind., to L.H. and Clarice Worley Boaz. He was preceded in death by his wife, Nancy Boaz.

He retired from Family Physicians Associated, Wabash, in 1996. He was an ordained Presbyterian elder and a member of Wabash Presbyterian Church. He served on the board of directors of the Wabash County Hospital. He was a diplomate of the American Academy of Family Practice. He was a proud member of the Phi Gamma Delta fraternity at Indiana University. He enjoyed playing golf at the Tippecanoe and Wabash country clubs. He was a U.S. Army Medical Corps veteran.

Surviving are a daughter, Jennifer Robin (and spouse Joel) Singer, Carmel; a son, Bradley Boaz, Carmel; five grandchildren: Elizabeth Anne, Emily Janet and Alexander William Boaz and Julie Debra and Robert Justin Singer; his loving companion, Anne Rogers, Paris, Ky. He was preceded in death by his parents.

Services will be at 2 p.m. Friday at Eastlund Funeral Home, Harris Chapel, Syracuse, with Pastor Steven Mullin officiating. Burial will be in Oswego Cemetery.

Calling is two hours prior to the service Friday at the funeral home.
