Charges of reckless homicide have officially been filed and an arrest warrant issued for Tadeusz Dylak, 55, Chicago.
Dylak is the semi-truck driver who allegedly disregarded a stop light and collided with Douglas Raber's pickup truck and a sport utility vehicle at the intersection of U.S. 30 and Center Street, Warsaw, May 13.
Raber died May 19 at Parkview Hospital, Fort Wayne, from injuries suffered in the accident.
According to police and witness reports, Dylak nearly hit at least two other cars. After inspecting the tractor-trailer, a state trooper found nine transportation department violations, three of which would have designated Dylak "out of service."
A crash reconstructionist reported Dylak entered the intersection 8-10 seconds after the light had turned red, and approximately 15 seconds after it was yellow.
