One man was airlifted to Fort Wayne and another was taken to Kosciusko Community Hospital after a head-on collision Friday afternoon.

Kosciusko County Sheriff’s Dept. deputies and emergency personnel were dispatched to a “head-on” collision on CR 600N near CR 150W at approximately 3:47 p.m. Friday afternoon.

Officers and responders removed one of the drivers, Jacob E. Polk, 25, of 265 S. East Ridge Drive, Warsaw, with serious leg injuries and immediately airlifted him to Parkview Memorial Hospital, Fort Wayne. Polk is in serious condition as of this morning.

The other driver, Robert P. Ferguson, 63, of 1642 W. CR 600N, Leesburg, was taken to Kosciusko Community Hospital, Warsaw, with minor bleeding to his head. Ferguson was treated and released from KCH.

According to Ferguson, he was eastbound on CR 600N in his 2007 GMC Sierra pickup truck and began to slow as he approached his residence.  He stated that he was viewing work that was being completed on his home when he turned into the path of a 2010 Toyota Corolla driven by Polk, who was westbound.

The Kosciusko County Fatal Alcohol Crash Team was called to measure and process the scene.

The crash is still currently under investigation by FACT at this time.
