Truck Driver Faces Charges In Fatal Crash

Tadeusz Dylak, the semi-truck driver allegedly responsible for causing the May 13 accident that killed Douglas Raber, was arrested on a reckless homicide charge Wednesday. His bond was set at $10,000.

Dylak, 55, Chicago, turned himself in at 8:46 a.m. Wednesday and bonded out soon after.

Dylak allegedly ran a stop sign at the intersection of U.S. 30 and Center Street, Warsaw, and collided with Raber's pickup and one other vehicle.

Raber died six days later in Parkview Hospital, Fort Wayne, from injuries suffered in the accident.

According to police reports, Dylak's vehicle should have been designated "out of service" prior to the accident because upon inspection, nine transportation department violations were found.
