SUMMONS FOR SERVICE BY PUBLICATION & NOTICE OF TERMINATION OF PARENTAL RIGHTS HEARING STATE OF INDIANA SS: COUNTY OF KOSCIUSKO IN THE MATTER OF THE TERMINATION OF THE PARENT-CHILD RELATIONSHIP OF: ZACHARIA HOLBROOK, Minor Child, And TARA HOLBROOK, Mother, JERAMIAH ECKERT, Father IN THE KOSCIUSKO COUNTY SUPERIOR COURT I, JUVENILE DIVISION WARSAW, INDIANA Cause No.: 43D01-1409-JT-298 TO: Jeramiah Eckert NOTICE IS HEREBY GIVEN to Jeramiah Eckert, whose whereabouts are unknown, and who is the father of Zacharia Holbrook (date of birth: March 21, 2006), that a Petition for Involuntary Termination of your Parental Rights of the above-named child, has been filed by the Indiana Department of Child Services, Kosciusko County Office, in the Kosciusko County Superior Court I, and YOU ARE HEREBY COMMANDED TO APPEAR before the Judge of said Court at the Kosciusko Superior Court 1, Juvenile Division, in the Kosciusko County Justice Building, 121 North Lake Street, Warsaw, Indiana 46580, on the 26th day of November 2014 at 9:30 A.M., to attend an Initial hearing/Termination hearing and to answer the Petition for Termination of your Parental Rights of said child, and YOU ARE FURTHER NOTIFIED that if the allegations in said petition are true, and/or if you fail to appear at the hearing, the Juvenile Court may terminate your parent-child relationship; and if the court terminates your parent-child relationship you will lose all parental rights, powers, privileges, immunities, duties and obligations including any rights to custody, control, visitation, or support of said child; and if the court terminates your parent-child relationship, it will be permanently terminated, and thereafter you may not contest an adoption or other placement of said child, and YOU ARE ENTITLED TO REPRESENTATION BY AN ATTORNEY, provided by the State if necessary, throughout these proceedings to terminate the parent-child relationship. YOU MUST RESPOND by appearing in person or by attorney within thirty (30) days after the last publication of this notice, and in the event you fail to do so, an adjudication on said petition and termination of your parental rights may be entered against you without further notice.
