SUMMONS BY PUBLICATION STATE OF INDIANA SS COUNTY OF KOSCIUSKO RLW ACCOUNTS, LLC, Plaintiff vs. CINDYLOU F. RUGGLES, Defendant IN THE KOSCIUSKO SUPERIOR COURT CAUSE NO. 43D03-0809-CC-876 COMES NOW the Plaintiff, RLW Accounts, LLC, by counsel, Rothberg, Logan & Warsco LLP, having filed its Complaint and Affidavit in the above-entitled cause, hereby requests that the Clerk of Kosciusko Superior Court, issue the Summons by publication as follows: 1. This Summons by publication is specifically directed to Defendant, Cindylou F. Ruggles, whose last known address was and is: 8280 US East 30 Lot 95 Pierceton, IN 46562, her present address is unknown. 2. The nature of the above-captioned cause is that on April 19, 1996, Defendant, Cincylou F. Ruggles executed and entered into a Cardholder Agreement. As of March 24, 2009, the principal balance owing is $4,454.67, plus accrued interest in the amount of $1,327.86, for a total amount owing of $5,782.53, plus accruing interest and costs. 3. RLW has accelerated the entire balance due and owing on the Agreement, due to Defendant's failure to make the required payments and is also responsible for prejudgment interest as set forth in the Agreement. 4. The Defendant, Cindylou F. Ruggles, named herein must answer the Summons in writing, personally or by counsel, on or before thirty (30) days from the date of the third publication hereof.
