Shawn M. Hazelet-Eiden, 38, of Angola, died at 8:55 a.m. Oct. 26, 2009, in Parkview Memorial Hospital, Fort Wayne.
She was born Dec. 5, 1970, in Warsaw, to Mark and Jackie Dodge Hazelet, who survive in Warsaw.
She lived most of her life in Warsaw, recently moving to Angola. She was a social worker and a member of St. Anthony Catholic Church, Angola. She did volunteer work at CAVA; Sus Amigos; YMCA, Angola; and St. Joe School, Garrett.
Also surviving are four children, Anthony Habermann, Warsaw, and Stephanie, Lawrence "Buck" and William "Wink" Eiden, all of Angola; a granddaughter, Kalli Daniel Slone; a sister, Tracey (and spouse Rob) Sleighter, Pierceton; two nieces, Shannon and Morgan Sleighter; a nephew, Trevor Sleighter; many loving aunts; an uncle; cousins; and two wonderful friends, Petty Jansen, the Netherlands, and Stephanie Garrett McDairmant, Warsaw.
Services will be at 11 a.m. Thursday at McHatton-Sadler Funeral Chapel, Warsaw, with Father Fred Pasche officiating. Burial will be in Leesburg Cemetery.
Calling is from 3 to 8 p.m. Wednesday at the funeral home.
