Scott D. Kreis, 35, of Warsaw, died at 2:28 a.m. June 16, 2009, in Kosciusko Community Hospital, Warsaw.
He was born Dec. 12, 1973, in Warsaw, to Greg and Carol Clark Kreis.
He was a lifetime Kosciusko County resident.
Surviving are his mother, Carol Kreis, Warsaw; his father, Greg Kreis, Rochester; a brother, Chad Kreis, Warsaw; and grandparents, Kathrin Clark, Warsaw, and Burton and Roberta Kreis, Claypool. He was preceded in death by a grandfather.
Private services will be at a later date.
McHatton-Sadler Funeral Chapel, Warsaw, is in charge of arrangements.
