Personal Injury Accident n 1:05 p.m. Monday – Lake City Highway, 2,640 feet east of South CR 250E, Warsaw. Two-vehicle collision. Drivers: Vicky L. Banks, 57, West US 30, Warsaw; Steven E. Shelmadine, 42, Columbia City. Injuries: Banks was transported to the hospital to be treated for foot pain. Damages: $2,500 to $5,000. Warsaw Police Department investigated.
