Levi Lee Road, Warsaw, arrested for operating a motor vehicle while intoxicated resulting in serious bodily injury and on a warrant in a civil matter. Bond: $12,250. • 12:44 p.m. Monday – Ashley Nicole Lyon, 21, of 10532 S. School Ave., Silver Lake, arrested for driving while suspended. Bond: $300. • 3:09 p.m. Monday – Kelly Ann Wagner, 42, Fort Wayne, arrested for failure to appear. Bond: $250. • 3:16 p.m. Monday – Ar Ya Du La, 25, Fort Wayne, arrested for public intoxication. Personal Injury Accident • 2:20 p.m. Monday – Buffalo Street, Warsaw. Vehicle slid on gravel. Driver: Justin H. Mickem, 33, Blaine Street, North Webster. Injuries: Mickem drove himself to Kosciusko Community Hospital because of an abrasion on his knee. Damages: Under $750. Warsaw Police Department investigated. WPD. • 8:35 p.m. Monday – Commerce Drive, Warsaw. Two-vehicle collision. Drivers: George M. Siddons, 48, Madden Road, Churubusco; Arianna E. Rossi, 23, Fort Wayne Street, Warsaw. Damages: $2,500 to $5,000. WPD. • 9:26 p.m. Monday – Intersection of Main and Detroit streets, Warsaw. Two-vehicle collision. Drivers: Julie C. White, 48, Hawthorn Road, Plymouth; Teresa L.
