Bond: $5,000. • 10:47 a.m. Friday – Michael S. Helvie, 33, Logansport, arrested for failure to appear – misdemeanor and check fraud – $750 to $50,000. Personal Injury Accidents • 5:53 p.m. Thursday – Intersection of North Ind. 15 and West Plum Street, Leesburg. Vehicle left roadway and rolled over into a ditch. Driver: A Din, 45, Fort Wayne. Injuries: Passenger Ka Kyi, 22, Fort Wayne, complained of fracture/dislocation of shoulder/upper arm; passenger Ha Shin, 56, Fort Wayne, complained of neck pain. The two were transported by ambulance to Kosciusko Community Hospital and Lutheran Hospital, Fort Wayne Damages: $2,500 to $5,000. Kosciusko County Sheriff’s Department investigated. • 5:40 a.m. Friday – Intersection of North Ind. 15 and West CR 800N, Warsaw. Two-vehicle collision. Drivers: Jack L. Mattingly, 47, West CR 250S, Warsaw; Dawn L. Hampton, 42, Sheridan Street, Warsaw. Injuries: Hampton and a passenger in her vehicle, Penny M. Ousley, 55, North Nancy Drive, Warsaw, both complained of arm pain and were checked at the scene by medics. Damages: $2,500 to $5,000. KCSD. • 7:15 a.m. Friday – North CR 200E, 1,000 feet north of East CR 550N, Leesburg. Vehicle left roadway and stopped in a field. Driver: Elizabeth S. Fleckenstein, 23, North Main Street, North Webster. Injuries: Fleckenstein complained of pain to the hip/upper leg but refused treatment at the scene. Damages: $5,000 to $10,000.
