Bond: $350. n 4:54 p.m. Tuesday – Emmaline Elizabeth Templeton, 21, Clarksville, Tenn., arrested for forgery. Bond: $5,000. n 5:25 p.m. Tuesday – John Matthew Seitz, 32, of 5966 W. Killdeer Drive, Mentone, arrested for failure to appear. Bond: $1,000. n 9:04 p.m. Tuesday – David Saint, 35, of 212 N. Franklin St., Mentone, arrested for criminal recklessness. Bond: $1,000.
Personal Injury Accidents n 4:40 p.m. Tuesday – 2605 E. Center St., Warsaw. Driver lost control and fell off scooter. Driver: Kenneth W. Callahan, 53, Kewanna. Injuries: Callahan was treated and released by EMS for abrasions to his right leg and arm. Damages: less than $750. Warsaw Police Department investigated. n 6:42 p.m. Tuesday – Lake City Highway, five feet west of CR 200N, Warsaw. Two-vehicle collision. Drivers: Richard L. Swartz, 69, Garrett; Ervin W. Monk, 45, Manitowoc, Wis. Injuries: Monk complained of neck pain. Damages: $10,000 to $25,000. Kosciusko County Sheriff’s Department investigated.
