PUBLIC NOTICE INDIANA DEPARTMENT OF ENVIRONMENTAL MANAGEMENT ELIGIBILITY FOR CONCENTRATED ANIMAL FEEDING OPERATION NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM GENERAL PERMIT Name and Address of Applicant: Maple Leaf Farms - Milford Farm Farm ID# 3175 P. O. Box 308 Milford, Indiana 46542 Location of Facility Applying for Permit: Near the junction of County Road 900 North & County Road 200 East at approximately 9166 North-CR-200 East, Milford, Indiana, 46542, in Section 26, Township 34 North, Range 6 East, in Kosciusko County. Background and Applicable Regulations The above-named applicant operates an existing five (5) unit IDEM previously approved duck confinement operation that confines 26,600 ducks in a dry manure handling system and 9,000 ducks in a liquid manure handling system. The applicant has submitted an application and a Notice of Intent (NOI) for a National Pollutant Discharge Elimination System (NPDES) Concentrated Animal feeding Operation (CAFO) general permit to comply with IC 13-18-10 and 327 lAC 15-15. Public Review and Comment This application is available for review in the Office of Land Quality, 100 North Senate Avenue, Indianapolis, Indiana, Monday through Friday, excluding official holidays, between the hours of 8.:15 a.m. and 4:00 p.m. E.S.T.
