IN THE KOSCIUSKO CIRCUIT COURT CAUSE NO. 43C01-1001-MI-53 To: Any Interested Person You are hereby notified that a Petition has been filed in the Kosciusko Circuit Court under Cause No. 43C01-1001-MI-53 whereby Petitioners DAVID WAYNE JOHNSTON and JEANINE THERESE JOHNSTON have requested that their name and the names of their minor children be changed from David Wayne Johnston to David Wayne DiVaccaro; Jeanine Theresa to Jeanine Theresa DiVaccaro; Vincent David Johnston to Vincent David DiVaccaro; Francesca Theresa Johnston to Francesca Theresa DiVaccaro; and Nicholas Luca Johnston to Nicholas Luca DiVaccaro. Said Petition was filed on the 29th day of January, 2010. Any person objecting to said name changes should appear at the hearing and file written objections thereto. Hearing in this cause will be conducted before the Kosciusko County Circuit Court in the Kosciusko County Justice Building at 121 N.
