PETITION FOR THE INVOLUNTARY TERMINATION OF THE PARENT-CHILD RELATIONSHIP STATE OF INDIANA SS COUNTY OF KOSCIUSKO IN RE: THE MATTER OF THE TERMINATION OF THE PARENT-CHILD RELATIONSHIP OF GABRIEL HETTINGER, the child, and TASHA PARKER, the child's mother, and DAVID HETTINGER, the child's father. KOSCIUSKO SUPERIOR COURT NO. 1 CAUSE NO. 43D01-0908-JT-379 The Indiana Department of Child Services, local office of Kosciusko County, by counsel, petitions the Court for an order terminating the parent-child relationship of Gabriel Hettinger, the child, and Tasha Parker, the child's mother, and David Hettinger, the child's father. The Department alleges: 1. The child has been removed from her parent's care and has been under the supervision of the Department of Child Services for at least six (6) months under a dispositional decree entered on November 20, 2008, in Cause No. 43D01-0809-JC-392. 2. There is a reasonable probability that the conditions that resulted in the child's removal will not be remedied and the continuation of the parent-child relationship poses a threat to the well-being of the child. 3. The termination of the parent-child relationship is in the best interest of the child. 4. There is a satisfactory plan for the care and treatment of the child. 5. No factors exist that would require dismissal of this termination petition pursuant to IC 31-35-2-4.5. WHEREFORE, the petitioner prays that the Court set this matter for a hearing to terminate the parent-child relationship of Gabriel Hettinger, the child, and Tasha Parker, the child's mother, and David Hettinger, the child's father, and for all other relief just and proper in the premises.
