Paul Evans, 64, formerly of Warsaw, died at 6:15 p.m. Tuesday, Feb. 12, 2019, in Life Care Center of Fort Wayne.
He was born Aug. 14, 1954, in San Bernardino, Calif., to Melville Clifton and Etta Francis Umel Evans.
He was united in marriage to V. Elaine Cook in Warsaw on April 5, 1986. She died July 9, 2011.
At his request, no services are planned.
Interment will be with his wife, Elaine, in Cook’s Chapel Cemetery, Warsaw.
