NOTICE TO NATURAL FATHER

STATE OF INDIANA

SS:

KOSCIUSKO COUNTY

IN THE MATTER OF THE ADOPTION OF

NATALIE DIANN SCHUE, minor

IN THE KOSCIUSKO SUPERIOR COURT #1

CAUSE NO. 43D01-1812-AD-41

    TO: Jason M. Schue

    Jason M. Schue, who is the natural father of Natalie D. Schue, born on the 3rd day of December, 2002, or who claims to be the father, IS HEREBY NOTIFIED that a Petition for Adoption of the child was filed in the office of the Clerk of Kosciusko Circuit Court, 121 North Lake Street, Warsaw, Indiana  46580.  The Petition for Adoption alleges that the consent to adoption of Jason M. Schue is not required because he has not had any substantial contact with Natalie D. Schue for a period of more than one year, and has not contributed to the support of Natalie D. Schue for a period of more than one year.

    If Jason M. Schue wishes to contest the adoption of the child, he must file a Motion to Contest the adoption in accordance with I.C. §31-19-10-1 in the above-named Court not later than thirty (30) days after the date of service of this Notice by publication.

    If Jason M. Schue does not file a Motion to Contest the adoption within thirty (30) days after service of this notice, the above-named Court will hear and determine the Petition for Adoption.  His consent will be irrevocably implied, and he will lose his right to contest either the adoption or the validity of his implied consent to the adoption.

    Nothing Jason M. Schue or anyone else may say to Natalie D. Schue relieves him of his obligation under this Notice.

    Under Indiana law, a putative father is a person who is named as or claims that he may be the father of a child born out-of-wedlock, but who has not yet been legally proven to be the child’s father.  For purposes of this Notice, Jason M. Schue is a putative father under the laws of Indiana regarding adoption.

    This Notice complies with I.C. §31-19-4-5, but does not exhaustively set forth a putative father’s legal obligations under the Indiana adoption statutes.  A person being served with this Notice should consult the Indiana adoption statutes.
