Fabricio Javier Hernandez and Angela A. Carbajal, who are the parents of the above named children born on February 5, 2013, is notified that a petition for adoption of the children was filed in the office of the Clerk of Kosciusko County, Indiana.
If you seek to contest the adoption of the children, you must file a motion to contest the adoption in accordance with I.C. 31-19-10-1 in the above-named court.
If you do not file a motion to contest the adoption, the above-named court will hear and determine the petition for adoption. Your consent will be irrevocably implied and you will lose your right to contest either the adoption or the validity of your implied consent to the adoption.
Nothing the Petitioners or anyone else says to either of you relieves you of your obligations under this notice.
