NOTICE TO NAMED FATHER STATE OF INDIANA SS: COUNTY OF KOSCIUSKO IN THE MATTER OF THE ADOPTION OF: DUSTIN TYLER HAWKINS AND LANE TRAVIS HAWKINS, minor children JUDY CAHILL AND JEREMY D. CAHILL, Petitioners IN THE SUPERIOR COURT NO. 1 OF KOSCIUSKO COUNTY CAUSE NUMBER 43D01-0909-AD-21 DANIEL L. HAWKINS, Who has been named the father of the children born to Judy Hawkins, now Cahill, on the 3rd day of June, 1993, and the 16th day of June, 1995, is notified that a Petition for Adoption of the children was filed in the Office of the Clerk of Superior Court No. 1 of Kosciusko County, Warsaw, Indiana. The Petition for Adoption alleges that Daniel L. Hawkins has abandoned and deserted said children. If Daniel L. Hawkins wishes to contest the adoption of the child, he must file a Motion To Contest Adoption in accordance with Indiana Code §31-19-10-1 et seq. in the above-named Court within thirty (30) days after the date of service of this Notice. If Daniel L. Hawkins does not file a Motion To Contest the adoption within thirty (30) days after service of this Notice, the above-named Court will hear and determine the Petition for Adoption. His consent will be irrevocably implied and Daniel L. Hawkins will lose his right to contest either the adoption or the validity of his implied contest to this adoption. Byron J.
