NOTICE TO NAMED FATHER STATE OF INDIANA SS COUNTY OF KOSCIUSKO IN THE MATTER OF THE ADOPTION OF: LILY ANN REICHEL IN THE KOSCIUSKO CIRCUIT COURT 2015 TERM CAUSE NO. 43C01-1508-AD-20 Chase James Reichel is notified that a Petition for Adoption of a child, named Lily Ann Reichel, born to Elizabeth Marie Reichel and Chase James Reichel, born on July 10, 2010, was filed in the Office of the Clerk of the Kosciusko Circuit Court, 121 North Lake Street, Warsaw, Indiana 46580. If Chase James Reichel seeks to contest the adoption of the child, Lily Ann Reichel, he must file a motion to contest the adoption in accordance with IC §31-19-10-1 in the above named court not later than thirty (30) days after the date of service of this notice. If Chase James Reichel does not file a motion to contest the adoption within thirty (30) days after service of this notice, the above named court will hear and determine the petition for adoption. The consent to adoption of Lily Ann Reichel will be irrevocably implied and Chase James Reichel will lose the right to contest either with adoption of Lily Ann Reichel or the validity of Chase James Reichel's implied consent to the adoption. No oral statement made to or by Chase James Reichel does not eliminate Chase James Reichel of the obligations under the notice.
