NOTICE TO NAMED FATHER SUMMONS STATE OF INDIANA, KOSCIUSKO COUNTY IN THE KOSCIUSKO SUPERIOR COURT NO. 1 121 NORTH LAKE STREET, WARSAW, INDIANA 46580 IN RE THE ADOPTION OF: ZM CARRIE MCGUIRE-YOUNG DUSTIN YOUNG, Petitioners CAUSE NO. 43D01-1604-AD-16 Zachary McKenzie, who has been named the father of the child born to Carrie McGuire-Young and/or Carrie McGuire on July 20, 2016, or anyone who claims to be the father of the child born to Carrie McGuire-Young or Carrie McGuire is hereby notified that a petition for adoption of the child was filed in the Office of the Clerk of the Kosciusko County Indiana Superior Court No. 1 located at 121 N. Lake Street, Warsaw, Indiana 46580 on the 19th day of April, 2016. If Zachary McKenzie or any other person seeks to contest the adoption of the child, you must file a motion to contest the adoption in accordance with Indiana code 31-19-10-1 in the above named court not later than 30 days after the date of service of this notice. If Zachary Mckenzie does not file a motion to contest the adoption within 30 days after service within 30 days after service of this notice, the above-named court will hear and determine the petition for adoption in your absence. Your consent will be irrevocably implied and you will lose your right to contest either the adoption or the validity of your implied consent to the adoption. You will lose your right to establish paternity of the child under Indiana Code. Noting Carrie McGuire-Young or anyone else says to Zachary McKenzie relieves his obligations under this notice. Under Indiana law, a putative father is a person who is named as or claims that he may be the father of a child born out of wedlock but who has not yet been legally proven to be the child's father. For purposes of this notice, Zachary McKenzie is the father under the laws of Indiana regarding adoption. This notice complies with Indiana Code, but does not exhaustively set forth a father's legal obligations under the Indiana Adoption Statutes.
