NOTICE TO NAMED FATHER IN THE KOSCIUSKO CIRCUIT COURT KOSCIUSKO COUNTY, INDIANA IN THE MATTER OF THE ADOPTION OF ISABEL MAE REHBEIN and MARY LEE REHBEIN CAUSE NUMBER 43C01-1303-AD-13 TO: Ismael Garcia Carmona Ismael Garcia Carmona, who has been determined to be the father of the above named children born to Michelle May Rehbein, is notified that a petition for adoption of the children was filed in the office of the Clerk of Kosciusko County. If Ismael Garcia Carbona seeks to contest the adoption of the children, he must file a motion to contest the adoption in accordance with I.C. 31-19-10-1 in the above named court. If Ismael Garcia Carbona does not file a motion to contest the adoption within thirty (30) days after service of this notice the above named court will hear and determine the petition for adoption. His consent will be irrevocably implied and he will lose his right to contest either the adoption or the validity of his implied consent to the adoption. Nothing Michelle May Rehbein or anyone else says to Ismael Garcia Carbona relieves Ismael Garcia Carbona of his obligations under this notice. This notice complies with I.C. 31-19-4-4 but does not exhaustively set forth the father's legal obligations under the Indiana adoption statutes.
