NOTICE TO FATHER STATE OF INDIANA SS COUNTY OF DELAWARE IN THE MATTER OF THE ADOPTION OF MINOR CHILD IN THE DELAWARE CIRCUIT COURT NO. 4 CAUSE NO. 18C04-1003-AD-2 Mark Thomas Simmons II, father of a child born to Alyssa Irene Fortriede (formerly Ritter) ("Mother') on November 21, 2006, is notified that a petition for adoption of the child was filed in the office of the Clerk of the Delaware Circuit Court No. 4, 100 West Washington Street, Muncie, Indiana. If Mark Thomas Simmons II seeks to contest the adoption of the child, he must file a motion to contest the adoption in accordance with IC 31-19 in the above named court, not later than thirty (30) days after the date of the last publication this notice. If Mark Thomas Simmons II does not file a motion to contest the adoption within thirty (30) days of the date of the last publication of this notice, the above named court will hear and determine the petition for adoption. Mark Thomas Simmons II's consent will be irrevocably implied and he will lose his right to contest either the adoption or the validity of his implied consent to the adoption. Nothing Mother or anyone else says to Mark Thomas Simmons II relieves Mark Thomas Simmons II of his obligations under this notice. This notice complies with IC 31-19 but does not exhaustively set forth a father's legal obligations under the Indiana adoption statute.
