NOTICE OF PETITION FOR CHANGE OF NAME STATE OF INDIANA SS COUNTY OF KOSCIUSKO IN THE MATTER OF THE CHANGE OF NAME OF PATRICK STUART NORMAN, Petitioner IN THE KOSCIUSKO CIRCUIT COURT TO: Any interested person You are hereby notified that a Petition has been filed in the Kosciusko Circuit Court under Cause Number 43C01-1208-MI-96 whereby Petitioner, Patrick Stuart Norman, has requested his name be changed to "Patrick Stuart Jennings".
