NOTICE OF PETITION FOR CHANGE OF NAME STATE OF INDIANA SSCOUNTY OF KOSCIUSKO IN THE MATTER OF THE CHANGE OF NAME OF JARED CHARLES WESTERBECK, BY NEXT FRIEND, PATTI J. TAYLOR, Petitioner IN THE KOSCIUSKO CIRCUIT COURT CAUSE NO. 43C01-1207-MI-91 TO: Any interested person. You are hereby notified that a Petition has been filed in the Kosciusko Circuit Court under Cause Number 43C01-1207-MI-91 whereby Petitioner/Next friend and Jared Charles Westerbeck, have requested the name of Jared Charles Westerbeck be changed to "Jared Charles Taylor". Said petition was filed on the 26th day of July, 2012. Any person objecting to said name change may appear and file written objections thereto.
