In compliance with 327 IAC 15-5 (Rule 5), notice is hereby given that Trilogy Health Services, LLC, 303 N. Hurstbourne Parkway, Suite 200. Louisville, KY 40222 is submitting a Notice of Intent to the Indiana Department of Environmental Management of their intent to discharge storm water from construction activities associated with the Warsaw Senior Living Facility project located at 2695 Shelden Street, Plain Township, Kosciusko County, Indiana starting on approximately November 01, 2017. Runoff from the project site will discharge into the storm sewer system and ultimately discharge south into Tippecanoe River.
