NOTICE Phend & Brown, Inc., 367 E 1250N, Milford, IN 46542, dba Leesburg Sand & Gravel at 70 W 600N, Leesburg, IN 46538, is submitting a Notice of Intent to the Indiana Department of Environmental Management of our intent to comply with the requirements of 327 IAC 15-5-5 to discharge storm water from construction activities associated with the extraction of sand and gravel on and extension of our existing gravel pit on the East side of State Road 15, Leesburg, IN 46538.
