LEGAL NOTICE In compliance with 327 IAC 15-5 (Rule 5), notice is hereby given that MS Warsaw, 14390 Clay Terrace Blvd. #205, Carmel, IN 46032 is submitting a Notice of Intent to the Indiana Department of Environmental Management of their intent to discharge storm water from construction activities associated with the MS Warsaw Transitional Care Facility project located at 2695 Shelden Street, Plain Township, Kosciusko County, Indiana starting on approximately May 15, 2015. Runoff from the project site will discharge into the storm sewer system and ultimately discharge south into Tippecanoe River.
