NOTICE OF INTENT - MIDWEST TRENCHLESS SERVICES INC

NOTICE Midwest Trenchless Service Inc. (351 W. 136th St. Grant, Michigan, 49327) is submitting a Notice of Intent to the Indiana Department of Environmental Management of our intent to comply with the requirements of 327 IAC 15-5 to discharge storm water from construction activities associated with The Town of Leesburg's Wastewater Collection and Pumping System located along SR 15 from W. Levi Lee Road to Plum St. and along all streets and alleys within the Town of Leesburg, Indiana 46538. Runoff from the project site will discharge to the Kohler Ditch and the Tippecanoe River.
