NOTICE OF HEARING ON PETITION REQUESTING TERMINATION OF TRUST AND DISTRIBUTION OF TRUST PROPERTY STATE OF INDIANA SS: COUNTY OF KOSCIUSKO IN RE: THE LEONARD E. WISE AND LENNIS C. WISE REVOCABLE LIVING TRUST IN THE KOSCIUSKO CIRCUIT COURT CAUSE NO. 43C01-1110-TR-133 Notice is hereby given to any and all beneficiaries, creditors, and any other persons claiming any interest in the Leonard E. Wise and Lennis C. Wise Revocable Living Trust. You are hereby notified that Douglas L. Wise, Trustee of the Leonard E. Wise and Lennis C. Wise Revocable Living Trust, has filed his Trustee's Petition Requesting Termination of Trust and Distribution of Trust Property, a copy of which is attached hereto. The same shall be heard in the Kosciusko Circuit Court, 121 N. Lake street, Warsaw, Indiana, on December 9, 2011, at 9:00 o'clock AM. You are further notified that the Court will, at the time of the hearing, enter its decree determining whether to terminate the Leonard E. Wise and Lennis C. Wise Revocable Living Trust, and, if so, the manner in which the assets held in the Trust are to be distributed. If you have any objections to the said Petition, you must file your written objections with the Court prior to the date of the hearing or appear at the hearing and state your objections to the Court; otherwise, the said Petition and matters alleged therein will be heard and determined in your absence.
