You are hereby notified that a Verified Petition for Legal Separation has been filed in the Kosciusko Superior Court 1, a copy of which is attached hereto and incorporated herein, and that the Court has set said Motion for Provisional Orders for hearing before the Court in the Kosciusko County Justice Building, 121 North Lake Street, Warsaw, Indiana at 9:00 am on the 12th day of January, 2018.
The parties are ordered to personally appear before the Kosciusko Superior Court 1 on said date and time.
