NOTICE OF ADOPTION IN THE ALLEN SUPERIOR COURT ADOPTON NO. 02D08-1701-AD-8 STATE OF INDIANA SS: COUNTY OF ALLEN IN THE MATTER OF THE PETITION FOR ADOPTION OF MINOR LANDON RYAN CARTER WAGONER MICHAEL AARON SEAN HALE, Adopting Parent Zachary Wagoner is notified that a Petition for Adoption of a child, named Landon Ryan Carter Wagoner, born to Kara Marie Hale on April 28, 2009, was filed in the office of the clerk of Allen Superior Court, Fort Wayne, Indiana. The Petition for Adoption alleges that the consent to adoption of Zachary Wagoner is not required because he had failed to support or communicate significantly with the child for a period of at least one (1) year. If Zachary Wagoner seeks to contest the adoption of the child, Zachary Wagoner must file a motion to contest the adoption in accordance with IC 31-19-10-1 n the above named court not later than thirty (30) days after the date of service of this notice. If Zachary Wagoner does not file a motion to contest the adoption within thirty (30) days after the service of this notice, the above named court will hear and determine the Petition for Adoption. The consent to adoption of Zachary Wagoner will be irrevocably implied and Zachary Wagoner will lose the right to contest either the adoption or the validity of Zachary Wagoner's implied consent to the adoption. No oral statement made to Zachary Wagoner relieves Zachary Wagoner of Zachary Wagoner's obligations under this notice. This notice complies with IC 31-19-4.5-3 but does not exhaustively set forth a person's legal obligations under the Indiana adoption statutes.
