"Bud" Brickey is notified that a petition for adoption of a child named Aryanna Jane Brickey, born to Jennifer Yates on May 21, 2012, was filed in the office of the Kosciusko Circuit Court, 121 N. Lake Street, Warsaw, Indiana. The petition for adoption alleges that the consent to adoption of Roy E. "Bud" Brickey is not required because since birth said Individual has completely and wholly abandoned the minor child for a period in excess of one year.

The petition for adoption alleges that the consent to adoption of Roy E. "Bud" Brickey seeks to contest the adoption of the child, Roy E. "Bud" Brickey must file a motion to contest the adoption in accordance with IC 31-19-10-1 in the above named court not later than thirty (30) days after the date of service of this notice. If Roy E. "Bud" Brickey does not file a motion to contest the adoption within thirty (30) days after service of this notice the above named court will hear and determine the petition for adoption. The consent to adoption of Roy E. "Bud" Brickey will be irrevocably implied Roy E. "Bud" Brickey will lose the right to contest either the adoption or the validity Roy E. "Bud" Brickey's implied consent to the adoption. No oral statement made to Roy E. "Bud" Brickey relieves Roy E. "Bud" Brickey of Roy E. "Bud" Brickey's obligations under this notice.
