NOTICE OF ADOPTION IN THE HAMILTON SUPERIOR COURT ROOM NO. 1 CAUSE NO. 29D01-1002-AD-325 STATE OF INDIANA SS: COUNTY OF HAMILTON IN RE: THE MATTER OF THE ADOPTION OF BRODIEY ALLWYN ROSE and ISRAEL OCTOBER ROSE minor children Heather Marie Rose is notified that a petition for adoption of children named, Brodiey Allwyn Rose, born July 18, 2005, and Israel October Rose, born April 5, 2006, was filed in the office of the Clerk the Hamilton Superior Court, Room No., 1, One Hamilton County Square, Noblesville, Indiana 46060. The petition for adoption alleges that the consent of Heather Marie Rose is not required pursuant to Indiana Code § 31-19-9-8. If Heather Marie Rose seeks to contest the adoption of the children, Heather Marie Rose must file a motion to contest the adoption in accordance with Indiana Code § 31-19-10-1 in the above named court not later than thirty (30) days after the date of service of this notice. If Heather Marie Rose does not file a Motion To Contest the adoption within thirty (30) days after service of this notice, the above named court will hear and determine the petition for adoption, the consent of Heather Marie Rose will be irrevocably implied, and Heather Marie Rose will lose her right to contest either the adoption or the validity of Heather Marie Rose's implied consent to the adoption. No oral statements made to Heather Marie Rose relieves Heather Marie Rose of Heather Marie Rose's obligations under this notice. This notice complies with Indiana Code § 31-19-4.5-3 but does not exhaustively set forth a person's legal obligations under the Indiana adoption statutes.
