Heidi R. Meinert is notified that a Petition for Adoption of a child, namely, Gauge Meinert, born to Heidi R. Meinert and Matthew Meinert on November 29, 2009, was filed in the office of the Clerk of the Kosciusko Superior Court 1, 121 North Lake Street, Warsaw, Indiana 46580. The Petition for Adoption alleges that the consent to adoption of Gauge Meinert is not required from Heidi R. Meinert because for a period of at least one year, she has failed without justifiable cause to communicate significantly with Gauge Meinert when able to do so and/or she has failed to provide for the care and support of Gauge Meinert when able to do so as required by law or judicial decree.
If Heidi R. Meinert seeks to contest the adoption of the child, namely, Gauge Meinert, then Heidi R. Meinert must file a motion to contest the adoption in accordance with I.C. 31-19-10-1 in the above named Court not later than 30 days after the date of service of this notice.
If Heidi R. Meinert does not file a motion to contest the adoption within 30 days after service of this notice, the above-named Court will hear and determine the Petition for Adoption. The consent to adoption of Gauge Meinert will be irrevocably implied and Heidi R. Meinert will lose the right to contest either the adoption or the validity of Heidi R. Meinert's implied consent to the adoption.
No oral statement to Heidi R. Meinert relieves Heidi R. Meinert of Heidi R. Meinert's obligation under this notice.
