NOTICE OF ADOPTION IN THE KOSCIUSKO SUPERIOR COURT 1 KOSCIUSKO COUNTY, INDIANA IN THE MATTER OF THE ADOPTION OF MARIN ELLE BELLAMY NEIL WATSON BRUNS, Petitioner CASE NO. 43D01-1007-AD-16 Brandon John Bellamy is notified that a Petition for Adoption of a child, namely, Marin Elle Bellamy, born to Heidi Renee Hawkins on September 18, 2003, was filed in the office of the Clerk of the Kosciusko Superior Court 1, 121 North Lake Street, Warsaw, Indianan 46580. If Brandon John Bellamy seeks to contest the adoption of the child, namely, Marin Elle Bellamy, then Brandon John Bellamy must file a motion to contest the adoption in accordance with I.C. 31-19-10-1 in the abovenamed Court not later than 30 days after the date of service of this notice. If Brandon John Bellamy does not file a motion to contest the adoption within 30 days after service of this notice, the above-named Court will hear and determine the Petition for Adoption. The consent to adoption of Brandon John Bellamy will be irrevocably implied and Brandon John Bellamy will lose the right to contest either the adoption or the validity of Brandon John Bellamy's implied consent to the adoption. No oral statement to Brandon John Bellamy relieves Brandon John Bellamy of Brandon John Bellamy's obligation under this notice. This notice complies with I.C. 31-19-4.5-3 but does not exhaustively set forth a person's legal obligations under the Indiana adoption statutes.
