IN THE KOSCIUSKO SUPERIOR COURT I KOSCIUSKO COUNTY, INDIANA
IN THE MATTER OF THE ADOPTION OF
QUENTIN OSBORN
TONY L. MEINERT
and JANEEN R. MEINERT, Petitioner
CASE NO: 43D01-1806-AD-15
Heidi R. Meinert is notified that a Petition for Adoption of a child, namely, Quentin Osborn, born to Heidi R. Meinert and Joe Osborn on January 19, 2006, was filed in the office of the Clerk of the Kosciusko Superior Court 1, 121 North Lake Street, Warsaw, Indiana 46580. The Petition for Adoption alleges that the consent to adoption of Quentin Osborn is not required from Heidi R. Meinert because for a period of at least one year, she has failed without justifiable cause to communicate significantly with Quentin Osborn when able to do so and/or she has failed to pay support.
The Petition for Adoption alleges that the Consent to Adoption of Quentin Osborn is not required from Joe Osborn because for a period of approximately eleven years he has failed without justifiable cause to communicate significantly with Quentin Osborn when able to do so and he has failed to provide for the care and support of Quentin Osborn when able to do so as required by law or judicial decree.
If Heidi R. Meinert and/or Joe Osborn seek to contest the adoption of the child, namely, Quentin Osborn, then Heidi R. Meinert and Joe Osborn must file a motion to contest the adoption in accordance with I.C. 31-19-10-1 in the above-named Court not later than 30 days after the date of service of this notice.
If Heidi R. Meinert or Joe Osborn do not file a motion to contest the adoption within 30 days after service of this notice, the above-named Court will hear and determine the Petition for Adoption.
The consent to adoption of Quentin Osborn will be irrevocably implied and Heidi R. Meinert will lose the right to contest either the adoption or the validity of Heidi R. Meinert's and Joe Osborn's implied consent to the adoption.
No oral statement to Heidi R. Meinert relieves Heidi R. Meinert of Heidi R. Meinert's obligation under this notice.
No oral statement to Joe Osborn relieves Joe Osborn of Joe Osborn's obligation under this notice. This notice complies with I.C. 31-19-4.5-3 but does not exhaustively set forth a person's legal obligations under the Indiana adoption statutes. A person being served with this notice should consult the Indiana adoption statutes.
