You are hereby notified that a Petition has been filed in the
Kosciusko Supreior Court I under Cause Numer 43D01-1712-MI-364 to
whereby Petition, Joshua Nathaniel Montel, has requested that his
name be changed to Joshua Nathaniel Level. Said petition was filed
on the 21st day of December, 2017. Any person objecting to said
name change may appear at the hearing and file written objections
thereto. Hearing in this cause will be conducted before the Kosciusko
Superior Court 1 in the Kosciusko County Justice Building, 121 North
Lake Street, Warsaw, Indiana 46580, on the 12th day of January, 2018,
at 9:30 o’clock a.m. Should you fail to appear the matter may be
heard and determined in you absence.
