NOTICE IS HEREBY GIVEN TO to all whom it may concern, that a Verified Petition for Name Change was filed by ELIZABETH GRACE CREEKMORE.
That the Petitioner was born on the 3rd day of February, 2014, in Kosciusko County, Indiana, and at the time of her birth her name was listed as “ELIZABETH GRACE CREEKMORE,” and that the Petitioner now desires to change her name to Elizabeth Grace Stapleton, as per her Verified Petition for Change of Name filed in this cause on the 31st day of May, 2017.
That a hearing will be held in connection with said request thirty (30) days following the final date of publication in this cause. Any interested persons must file an answer, in writing, in person or by attorney, within thirty (30) days, commencing the day after final publication of this notice, or this petition may be approved as requested in the petition filed in this Court, and said action will be heard and determined by the Judge of this Court on June 14, 2019, at 9:30 a.m., in the Kosciusko Circuit Court, Kosciusko, Indiana.
