NOTICE OF KOSCIUSKO COUNTY HEALTH DEPT. ORDER TO TAMMY HONEYCUTT AND MICHAEL WRIGHT STATE OF INDIANA ss: COUNTY OF KOSCIUSKO Notice is hereby given to Tammy Honeycutt and Michael Wright that the dwelling located at 8023 South Packerton Road, Claypool, Indiana, is HEREBY DECLARED to be unfit for human habitation pursuant to I.C. 16-41-20-1 due to an accumulation of animal waste within the premise, which not only renders the said dwelling uninhabitable but which also poses a significant health risk to neighboring homes. Furthermore, said dwelling is HEREBY DECLARED to be a public nuisance pursuant to I.C. 16-41-20-6. Accordingly, you are HEREBY ORDERED to cleanse and purify the residence located at 8023 South Packerton Road, Claypool, Indiana on or before the 23rd day of September, 2013. You may request that this Order be reviewed by filing a petition with the Kosciusko County Circuit or Superior Court pursuant to I.C. 16-41-20-9. If review is not requested within the time allowed by I.C. 16-41-20-9 (not more than ten (10) days after the making of this Order) and if this Order is not complied with prior to the expiration of the time given above, the Kosciusko County Health Department will initiate formal court proceedings to seek court approval to carry out the necessary action. Additionally, the Health Department may seek damages in the amount of all costs and expenses paid and/or incurred with respect to any failure to comply with this Order.
