NOTICE OF KOSCIUSKO COUNTY HEALTH DEPT. ORDER TO ANY AND ALL HEIRS AND DEVISEES OF NORMAN E. BUSZ AND ALSO TO ANY AND ALL OWNERS OR HOLDERS OF ANY INTEREST IN REAL PROPERTY COMMONLY KNOWN AS 5703 SOUTH LAKE SHORE DRIVE E, WARSAW, INDIANA 46580 STATE OF INDIANA ss: COUNTY OF KOSCIUSKO Notice is hereby given to any and all heirs and devisees of Norman E. Busz, who died on May 23, 2013, and also to any and all owners or holders of any interest in real property commonly known as 5703 South Lake Shore Drive E, Warsaw, Indiana 46580, which is located in Kosciusko County, Indiana. The aforementioned property is in violation of Kosciusko County Ordinance No. 08-11-10-001, due to unsealed doors allowing unauthorized entrance into the premises and also due to tires holding water, which may serve as a breeding site for mosquitoes. Accordingly, you are HEREBY ORDERED to seal all doors and properly dispose of tires located at the premises on or before the 31st day of July, 2014. You may request that this Order be reviewed by filing a petition with the Kosciusko County Circuit or Superior Court pursuant to I.C. 16-41-20-9. If review is not requested within the time allowed by I.C. 16-41-20-9 (not more than ten (10) days after the making of this Order) and if this Order is not complied with prior to the expiration of the time given above, the Kosciusko County Health Department will execute upon this Order by sealing the doors and properly disposing of the tires. Additionally, a civil action may be brought for all costs and expenses paid and/or incurred by the Kosciusko County Health Department in the execution of this Order. William F.
