NOTICE OF CONSTRUCTION ACTIVITY Kosciusko 21st Century Foundation, Inc. of 2170 North Point Drive, Warsaw, Indiana 46582 is submitting an NOI letter to notify the Indiana Department of Environmental Management of its intent to comply with the requirements under 327 IAC 15-5 to discharge storm water from construction activities for the following project: Medial Office Building located at 902 Provident Drive approximately 1,700 feet southeast of Springhill Road.
