SUMMONS FOR SERVICE BY PUBLICATION & NOTICE OF TERMINATION OF PARENTAL RIGHTS HEARING STATE OF INDIANA SS COUNTY OF KOSCIUSKO IN THE MATTER OF THE TERMINATION OF THE PARENT-CHILD RELATIONSHIP OF BRANTLEE STIMMEL-MEYER and (alleged Father) JAMES HAMPTON IN THE SUPERIOR 1 COURT JUVENILE DIVISION WARSAW, INDIANA CAUSE NO. 43D01-1011-JT-434 TO: NOTICE IS HEREBY GIVEN to James Hampton, whose whereabouts are unknown, and who is the alleged biological father of Brantlee Stimmel-Meyer (date of birth December 11, 2007, as well as any Unknown Alleged Father whose whereabouts are also unknown, that a Petition for Involuutary Termination of your Parental Rights in the above named Child(ren), attached hereto, has been filed by the Indiana Department of Child Services, Kosciusko County Office, in the Kosciusko County Superior 1 Court, and You HEREBY COMMANDED TO APPEAR before the Judge of said Court at the Courthouse, in Warsaw, Indiana, (telephone 574-372-2331), on the 28th day of February, 2011 at 9 o'clock A.M., to attend a Termination hearing and to answer the Petition for Termination of your Parental Rights in said Child(ren), and You ARE FURTHER NOTIFIED that if the allegations in said petition are true, and/or if you fail to appear at the hearing, the Juvenile Court may terminate your parent-child relationship; and if the court terminates your parent-child relationship you will lose all parental rights, powers, privileges, immunities, duties and obligations including any rights to custody, control, visitation, or support in said Child(ren); and if the court terminates your parent-child relationship, it will be permanently terminated, and thereafter you may not contest an adoption or other placement of said child(ren), and You ARE ENTITLED TO REPRESENTATION BY AN ATTORNEY, provided by the State if necessary, throughout this proceedings to terminate the parent-child relationship. You MUST RESPOND by appearing in person or by attorney within ten (10) days after the last publication of this notice, and in the event you fail to do so, an adjudication on said petition and termination of your parental rights may be entered against you without further notice.
