According to the police report filed Aug. 26 in Kosciusko Superior Court II, at 3 p.m. Aug. 17 central dispatch received a call from a woman who said a man was standing in the in the left turn lane of the westbound lanes of U.S. 30 in front of Arby’s and had pulled his pants down, exposing himself.

Warsaw police officers had prior run-ins with Cook for being belligerent and yelling at traffic.

When police located Cook on Frontage Road, Cook was acting erratic and appeared to be under the influence of drugs, the police report states.

Cook told police that if anyone saw his genitals, “they would have liked it,” according to the affidavit.

Police transported Cook to Parkview Hospital in Warsaw for a psychological evaluation, and according to court papers, police provided Parkview with a 24-hour emergency involuntary detention form.
