K21 Health Foundation currently is accepting applications for the final round of grants in 2009. Completed applications must be received no later than 4 p.m. Nov. 2 to be considered during this cycle.
Non-profit organizations with programs dedicated to the health and wellness of Kosciusko County residents are encouraged to apply. All applicants will be required to submit the recently redesigned grand application which is available at www.k21foundation.org
K21 Health Foundation exists for the benefit of Kosciusko County Citizens to ensure health care services are provided, and to advance prevention and healthy lifestyles. K21 is at 2170 N. Pointe Drive, in the Keystone Professional Center.
